  • I knowingly and voluntarily assume all risks, including those arising from the ordinary negligence of the Company and its managers, members, employees, and contractors

  • I release, waive, and covenant not to sue the Company to the fullest extent permitted under California law (excluding gross negligence or intentional misconduct)

  • I understand the Company does not provide medical advice

  • I agree to obtain written medical clearance before returning to training following any injury, including concussions

  • I assume all risks related to communicable diseases, including COVID-19

  • I grant the Company permission to use my photo, video, likeness, and voice for commercial and promotional purposes

  • I agree to binding arbitration under the California Arbitration Act (CCP §1280 et seq.) and waive my right to a jury trial

  • If I am signing on behalf of a minor, I accept full responsibility and agree to indemnify and defend the Company
